10/9/2014: Safety alert – Preventing barge explosions

The Coast Guard issued marine safety alert 10-14 to bring attention to recent casualties involving explosions aboard barges conducting tank cleaning operations alongside marine terminals have resulted in serious injuries to vessel crews and facility workers, catastrophic property damage and harm to the environment.

A review of related casualties revealed that vessel personnel, facility personnel and shore side managers failed to ensure established procedures and safe practices were followed.

The safety alert highlights critical lessons learned from these incidents.

The alert was developed by the Coast Guard’s Eighth District Prevention Division and the Office of Investigations and Casualty Analysis.
