5/12/2016: Lessons learned on the Outer Continental Shelf

The Outer Continental Shelf National Center of Expertise (OCSNCOE) published two reports on lessons learned from past events.

One of the reports covers the proper use of the Well Specific Operating Guide (WSOG) to drive operational decision making. The other covers an inadvertent button-push and risk identification in the Job Safety Analysis.
