The Coast Guard has has closed its investigation of a June 30, 2019, lifeboat incident on the Floating Outer Continental Shelf (OCS) Facility AUGER that resulted in the loss of two lives and a serious injury.
Marine casualty reporting forms updated
The CG-2692 series of forms for reporting marine casualties has been re-approved by the Office of Management and Budget and updated with the new expiration date of July 31, 2022. Owners, agents, masters, operators, or persons in charge of a vessel should begin using the updated versions of the CG-2692 series immediately.
12/16/2016: Safety alert – MAN B&W Cooling Jacket Failures
The Coast Guard published Marine Safety Advisory 17-16 to bring attention to a problem with cracks on the cooling jacket, specifically MAN B&W, that have been linked to steering and propulsion casualties. All vessel owners, operators and engineers are encouraged to view this alert.
